Hopkins Opioid Program (HOP)

Welcome to the Sidney Kimmel Comprehensive Cancer Center at Johns Hopkins Center for Cancer Pain Research’s opioid conversion program. We have designed this tool to facilitate appropriate conversions from one opioid or route of administration to another. As with any clinical tool, we strongly encourage clinicians to use the information provided within the context of each individual patient.

The authors hope you find the program valuable. Please remember to register so that we may keep you abreast of new versions of the program. Also, please provide any feedback on your experiences with the Hopkins Opioid Program.
